PT for Life offers concierge physical therapy services. Concierge physical therapy is one -on- one, high quality physical therapy which is specially tailored to each client’s individual needs.


WHO IS CONCIERGE PT FOR?

  • Concierge PT services are ideal for individuals who want to remain active for life.

  • A physician referral is not needed, but we are happy to communicate with your physician to enhance your continuity of care.

  • Private payment is required for Concierge PT Services, however some PPOs may reimburse, depending on your particular insurance plan.

  • Concierge PT services can be delivered in the clinic or in the privacy of your home.

IN THE CLINIC

$175  Physical Health and Wellness Evaluation

This is an hour long assessment of your past medical history, postural misalignments, muscle imbalances, strength, balance, flexibility and current injury or concerns. This baseline of your current physical status will be used to set realistic goals and create a personalized treatment plan to meet these specific goals.

$150 Physical Therapy Treatment Session

This 60 minute follow-up treatment session may include manual techniques, modalities, stretching, mobility, stability and strength training.

 $175 Physical Therapy and Wellness Evaluation
